Your Attention Is Needed for the Following

CYBER RISK MANAGEMENT

• Your risk analysis must be current. An outdated assessment will not hold up under an audit or enforcement review.

• Every system touching ePHI must be accounted for. Unreviewed access points are among the top findings in HIPAA enforcement actions.

• Risk management is a cycle, not a one-time task. Regulators expect ongoing identification and remediation. A single snapshot from years ago will not satisfy reviewers.

• If it is not documented, it did not happen. Policies, risk registers, and remediation records are the first things auditors ask for.

RANSOMWARE PREPAREDNESS

• Healthcare is the top ransomware target. The updated Security Rule expects written response and recovery plans. Awareness alone is not enough.

• Backup and recovery plans must be tested. Documented contingency procedures are a HIPAA requirement. Untested ones are a liability.

• Staff are your most exposed attack surface. Phishing drives most ransomware incidents. Regular training is both a compliance requirement and a practical defense.

• Response timelines are tightening. Know exactly how quickly your team must identify, contain, and report a breach under the updated rule.

VENDOR OVERSIGHT

• Any vendor touching ePHI needs a signed BAA. No agreement means no protection. There is no excuse if something goes wrong without one.

• A BAA is the floor, not the ceiling. If a vendor is breached and you have never reviewed their security practices, the agreement alone will not shield you.

• Maintain a vendor risk inventory. Document which vendors access ePHI, their risk level, and when they were last reviewed.

• Know when to act. A clear inventory lets you respond quickly when a contract changes, expires, or a security concern surfaces.
